Inpatient Referral Process
Inpatient referrals may be called to any of the following at West Florida Rehabilitation Institute:
Nurse Liaisons:

Records can be faxed to (850) 494-5124 to the attention of “Nurse Liaison.”
The need for an on-site visit is determined on an individual basis. When appropriate, the visit is made by the Rehabilitation Institute Liaison Nurse.
Insurance Coverage
Our Business Office will verify insurance coverage for rehabilitation. The patient or family may need to be contacted during this process. If there are questions, the Liaison Nurse will contact you.
