Registration
At West Florida Hospital, we want your registration process to go as smoothly as possible. The following should help answer any questions you may have. If not, just give us a call!
When you check in for inpatient or outpatient services, you may be required to sign the following:
- Consent for treatment
- Authorization to bill insurance
- If you are a Medicare recipient, you will be required to complete a:
- Medicare Secondary questionnaire; and
- Medicare Rights Statement
- If you are a Tricare recipient, you will be given a
- Tricare Rights Statement
- Remember to bring the following with you at the time of registration:
- Social Security Card
- All active insurance cards
- Picture ID
- Living Will, if you have one

Visiting Hours
Acute Care Patients: 9:00 a.m. - 8:30 p.m.
ICU, CCU and NINU: 9:00 - 9:30 a.m.; 1:00 - 1:30 p.m.; 5:00-5:30 p.m.; and 8:00 - 8:30 p.m.
Family Birthplace: 9:00 a.m. - 8:30 p.m.

Inpatient Registration Information
Admission
The Admitting Office is located on the first floor near the hospital's main entrance. When you are admitted to the hospital, we will enter your information into our computer and you will sign the necessary admission forms. You will then be escorted to your room by a patient transporter.
You need only bring essentials with you to the hospital. These might include pajamas, bath robe, and personal hygiene items. Bring your medications, too, if your physician has instructed you to do so.
Insurance
You should bring your insurance policy or insurance identification card when you are admitted.
Financial Arrangements
Financial arrangements are made in the Business Office (Patient Accounts) located on the first floor near the hospital's main entrance. Arrangements for payment of your hospital bill must be made prior to or at the time of admission. If you anticipate difficulty concerning the financing of your hospitalization, please contact your patient counselor in the Patient Accounts Office at 494-4120 prior to admission to make necessary arrangements.
