The Pavilion at West Florida Hospital is proud to offer Transitions which is a compassionate outpatient programs that support individuals on their journey to wellness. Transitions is tailored to meet individuals specific needs for support in an outpatient setting and includes our Partial Hospitalization Program and our Intensive Outpatient Program, which is offered in the am and afternoon.
Partial hospitalization program
Our Partial Hospitalization Program provides the highest level of intensity treatment available in an outpatient setting. It is designed for those who need increased support and treatment but do not require inpatient hospitalization. Generally, patients attend PHP 5 days a week, for 4 to 6 weeks.
Intensive outpatient program
The Intensive Outpatient Program is for individuals willing to attend group therapy and who actively participate in their treatment recovery. Each participant contributes to developing their individualized plan of treatment to meet their goals and achieve success. Individuals in the Intensive Outpatient Program attend less frequently than individuals who attend the Partial Hospitalization Program and will transition to follow up sessions with a private provider such as a psychiatrist, psychologist or licensed therapist. Generally, patients attend IOP 3 days a week for 8 to 10 weeks.
To qualify for admission to Transitions, individuals must:
- Have a Primary Behavioral Health Diagnosis
- Be Motivated for Group Therapy and Willing to Attend Therapy Sessions
- Participate and Benefit from Intensive Group Therapy
Screenings are confidential and are offered at no charge.
Transitions is available to individuals who are ages 18 and older and allows participants to receive group therapy at the Pavilion at West Florida Hospital and return to the comfort of their own homes.
Individuals are under the care of a psychiatrist and receive therapy from a licensed behavioral health professional. Therapy is available Monday through Friday and the frequency of attendance is tailored to individual needs. Participants may remain in each program as long as treatment is received for their behavioral health needs. Our goal is to provide each participant an individualized plan to attain wellness and stability with eventual discharge to a less structured treatment environment. All patients receive aftercare planning to support their next step to wellness.
Our outpatient programs are covered by Medicare. All insurance coverage will be verified prior to admission to assist patients in determining any out-of-pocket expense.
Participants are responsible for their own transportation to and from programs.
